Who we are
Dignity Living Solutions LLC (“Dignity,” “we,” “us,” or “our”) operates dignitylivingsolutions.com (the “Website”). We coordinate housing referrals for older adults, disadvantaged individuals and their families, work with residential Provider Partners and service vendors, provide management services, and help residents find services and resources.
This Privacy Policy explains what personal information we collect through the Website, our forms, phone calls, emails, and text messages, and how we use, share, and protect it. If you have questions, see “Contact us” at the end of this policy.
Information we collect
Information you give us
Contact information, such as your name, phone number, email address, mailing address, and preferred contact method and times.
Housing and care preferences, such as preferred area, monthly budget range, move-in timeline, room type, gender for shared housing, mobility, care level, memory care, dietary needs, smoking, and other needs relevant to finding suitable housing.
Information about another person. If you are a family member or caregiver seeking help for someone else, we collect the information you provide about that person.
Business information from Provider Partners and vendors, such as business name, contacts, license and insurance information, service areas, rates, availability, referral preferences, and billing and payment records.
Communications, such as form submissions, emails, text messages, call notes, and your consent choices.
Information we receive from others
EverHeart Senior Living, a related company, when you ask it or authorize it to connect you with us.
Referral sources, such as family members who contact us on your behalf.
Provider Partners and vendors, including updates on the status of a referral, such as whether a move-in occurred.
Information collected automatically
When you visit the Website, we and our website platform automatically collect information such as your IP address, browser and device type, pages viewed, the site that referred you, and the date and time of your visit, using cookies and similar technologies. See “Cookies and analytics.”
What not to send us
Please do not submit medical records, Social Security numbers, Medicare or Medicaid numbers, bank or card numbers, or financial statements through the Website or its forms. Provider Partners enter payment information directly with our payment processor, not through a Website form.
Sensitive information
Some information you share about care needs, such as mobility, memory care, or clinical needs, may be sensitive personal data under Texas law. We collect it only when you choose to provide it, and we use it only to identify suitable housing, services, or resources for you. We share it with a Provider Partner or vendor only with your consent.
If you submit information about another person, you confirm that you are authorized to share it and to make the request on that person’s behalf.
How we use information
We use personal information to:
respond to your inquiries and requests;
understand your housing needs and identify communities, services, or resources that may fit them;
make housing introductions and service introductions you authorize;
review and manage Provider Partner and vendor applications and relationships;
provide management and resident-service coordination;
send you messages about your inquiry, referral, or appointment, and marketing messages only if you choose to receive them;
invoice and collect fees from Provider Partners;
operate, secure, and improve the Website and our services; and
keep records and comply with legal obligations.
Our staff, not an automated system, decide which communities, services, or resources to suggest to you.
How our housing referral service works
Texas law requires referral agencies for senior living communities to tell consumers the following:
Our referral service is free to you. Provider Partners pay Dignity a referral fee when they accept a referral, and a placement fee if you move in.
We select communities based on your stated needs and preferences. Cost is never the only factor, and the fees a community pays do not affect which communities we suggest.
You may stop using our service at any time, without cause or penalty. If you do, we will let every community we referred you to know.
The communities we suggest may not include all communities in your area that meet your stated preferences and needs.
We do not refer you to any community in which Dignity has an ownership, management, or financial interest.
Before sharing your name or contact information with a Provider Partner, we first send the Provider Partner a summary that does not identify you, such as care level, county, and budget range. We release your name and contact information only after you have authorized us to do so.
How we share information
Provider Partners. With your authorization, we share the information a residential Provider Partner needs to evaluate and contact you. Provider Partners must use it only to evaluate, contact, and serve you, keep it confidential, and not sell it.
Service vendors and resources. With your authorization, we share the information needed to introduce you to an appropriate vendor or resource. A public resource listing is not an approved or paid vendor.
EverHeart Senior Living. We share information with EverHeart, a related company, when you ask about housing at EverHeart or are an EverHeart resident receiving coordinated services.
Service providers. We use companies that help us run our business, including customer relationship management and website hosting, email, phone and text messaging, electronic signature, payment processing, and IT support. They may use your information only to provide services to us.
Legal and safety reasons. We may disclose information to comply with law, a subpoena, or a court order; to protect the rights, property, or safety of any person; or to report suspected abuse, neglect, or exploitation as required by Texas law.
Business transfers. If Dignity is involved in a merger, sale, or reorganization, information may be transferred as part of that transaction, subject to this policy.
We do not sell personal information to data brokers, and we do not share it for targeted advertising. Disclosures to Provider Partners and vendors are made at your direction, to provide the service you requested.
Text messages
If you provide your mobile number and consent, Dignity Living Solutions may send you text messages about your inquiry, referral, appointments, and service updates. We send marketing text messages only if you separately opt in to receive them. Consent to receive text messages is not a condition of using our services.
Message frequency varies. Message and data rates may apply.
Reply STOP to any message to stop receiving texts. Reply HELP for help, or contact us using the information below.
Mobile carriers are not liable for delayed or undelivered messages.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging opt-in data and consent will not be shared with any third parties. This excludes only the service providers that deliver our messages.
Cookies and analytics
The Website uses cookies that are necessary to operate the site and its forms, and analytics cookies that help us understand how visitors use the site. You can block or delete cookies through your browser settings, but some forms may not work properly without them.
We treat a Global Privacy Control signal from your browser as a request to opt out of any sale of personal information or targeted advertising. Because there is no common standard for browser “Do Not Track” signals, we do not respond to them.
How long we keep information
We keep personal information only as long as needed for the purposes in this policy:
Housing inquiries and referral records: three years after the later of our last contact with you or the referral date.
Provider Partner and vendor records: for the relationship plus three years.
Invoices, payment, and tax records: seven years.
Text message consent and opt-out records: five years after the last message.
Website analytics: up to two years.
After these periods, we delete or de-identify the information, unless we must keep it longer to comply with law or resolve a dispute.
Your choices and rights
Wherever you live, you may ask us to:
confirm whether we process your personal information and give you access to it;
correct inaccurate information;
delete your information;
give you a copy of your information in a portable format;
opt out of any sale of your information, targeted advertising, or profiling; and
withdraw your consent to our use of sensitive information.
To make a request, email [email protected]. We will verify your identity before acting on a request. An authorized agent may make a request on your behalf with your written permission. We will respond within 45 days, and if we need more time we will tell you and may extend that period once by 45 days. Requests are free up to twice in any 12-month period.
If we deny your request, you may appeal by replying to our decision or emailing [email protected] with “Privacy Appeal” in the subject line. We will respond to your appeal within 60 days. If we deny your appeal, you may contact the Texas Attorney General at consumerprotection.texasattorneygeneral.gov.
We will not deny you service, charge you a different price, or treat you differently for exercising these rights. You may also unsubscribe from marketing emails using the link in any email, and stop text messages by replying STOP.
How we protect information
We protect personal information with reasonable administrative, technical, and physical safeguards. These include encrypted (HTTPS) connections for the Website and its forms, access limited to staff who need the information and who sign in with individual accounts, confidentiality training for staff who work with consumers, and confidentiality obligations for our service providers, Provider Partners, and vendors. Card payments are handled by our payment processor, and we do not store full card numbers.
No website or internet transmission is completely secure, so we cannot guarantee absolute security. If a security breach affects your personal information, we will notify you as required by Texas law.
Children
The Website and our services are not intended for anyone under 18, and we do not knowingly collect personal information from children. If you believe a child has given us personal information, contact us and we will delete it.
Links to other websites
The Website may link to websites we do not control, including those of Provider Partners, vendors, and resources. This policy does not apply to those websites. Please review their privacy policies.
Changes to this policy
We may update this Privacy Policy from time to time. We will post the updated policy on this page and change the “Last updated” date. If we make a material change to how we use or share personal information, we will post a notice on the Website or notify you by email before the change takes effect.
