Join a network of residential providers that Dignity refers older adults and disadvantaged individuals to: group homes, shared living, independent living, boarding homes and assisted living. Dignity also offers Vendor Access to the residents of its Provider Partners. Together we can help more people find the right place at the right time.

From application to first referral

  1. Apply for each residence

    Complete one application per residence. There is no application fee.

  2. Send your documents

    License or written exemption basis, and a certificate of general liability insurance.

  3. Sign the agreement

    Your organization signs one Referral Partner Agreement listing its approved residences.

  4. Dignity approves activation

    Only a complete application, signed agreement and Dignity approval make a residence active.

  5. Receive offers

    Accept or decline non-identifying summaries. Details are released only after the steps in your agreement.

Vendor Access Add-On

For $50 per enrolled location per month, your community can send resident service requests to Dignity, and we coordinate with businesses in our vendor network. Residents always choose their own providers and pay vendors directly, or through their own coverage where it applies.

  • Separate from housing referrals. Joining or declining it never affects your referrals.
  • One flat fee per location, billed monthly in advance. No per-request fees.
  • Pay by card, debit card or ACH. You can choose automatic payment when you enroll.

Provider Partner questions

Who can apply to be a Provider Partner?

Residential providers serving people seeking alternative housing may apply. Dignity reviews each organization, residence profile, service area, rates and current availability before activation.

Does submitting an application make us an active partner?

No. Dignity must receive a complete application and signed Referral Partner Agreement, then approve your organization before sending referral offers.

How do referral offers work?

Dignity first sends a non-identifying summary for you to accept or decline. Identifying information is released only after the applicable fee is paid, the individual has authorized sharing, and Dignity approves release.

Are there membership fees or referral fees?

There is no monthly membership fee under the current model. Applicable qualified lead and placement fees are disclosed in the signed Referral Partner Agreement before referral offers begin.

Who decides whether a person can move in?

Your organization makes its own admission decision and confirms its rates, services and available space. An introduction does not guarantee a move-in.
