When a person is not the right fit for an EverHeart community, or is looking for a different kind of home, Dignity can review their needs and introduce them to residential Provider Partners that may be a better match.

Homes we refer to

Residential group homes, shared living, independent living, boarding homes and assisted living providers that have applied, signed a Referral Partner Agreement and been approved by Dignity. Each provider makes its own admission decision.

How a referral works

  1. Tell us what you need

    Share location, budget, room preference, timing and daily needs on the request form.

  2. EverHeart first, when it fits

    If your needs fit an EverHeart Senior Living home, we connect you with EverHeart.

  3. We review and look for a fit

    Our staff, not an automated system, review providers whose homes may match.

  4. Providers see a summary first

    A provider first receives a summary without your name or contact information.

  5. Introduction with your permission

    We confirm with you before sharing identifying information with any provider.

  6. The provider decides

    You tour or talk with the provider. It confirms availability, rates and admission.

How our referral service works

Texas law requires us to tell you:

  • Our referral service is free to you. Participating housing providers pay Dignity a referral fee when they accept a referral and a placement fee if you move in.
  • We suggest providers based on your stated needs and preferences. Cost is never the only factor, and the fees a provider pays do not affect which providers we suggest.
  • The providers we suggest may not include every provider in your area that meets your preferences and needs.
  • You may stop using our service at any time, without cause or penalty.
  • We do not refer you to any provider in which Dignity has an ownership, management or financial interest.

Housing referral questions

How does Dignity help me find housing?

We review the housing preferences you share, identify residential Provider Partners that may fit, and coordinate an introduction if you authorize us to share your information.

Does Dignity make the admission decision?

No. Each residential provider confirms its own availability, rates, admission requirements and final decision.

Will my information be shared without my permission?

We may discuss a possible fit without identifying you. We share identifying information with a selected provider only after you authorize the introduction and Dignity completes its review.

What happens after an introduction?

Dignity may follow up to learn whether you connected with the provider, scheduled a tour or assessment, or moved in. You can tell us when your needs change.

What if I am unsure which housing type fits?

Submit the housing request form and tell us what matters most, including location, budget, room preference and timing. We can review possible options with you.
